Customer Account Information Form

We thank you for subscribing to our services! To enhance our ability to serve you, we kindly ask you to complete the following information on this form, which will be used to establish your new customer account on the Cloud One Portal.

We will create the necessary contacts on our portal and send credentials to the appropriate individuals.

  • Customer Accounts Payable
  • Customer Technical Support Lead

By having these contacts, we can streamline the invoicing process, improve helpdesk ticket management, efficiently create e-contracts, and generate effective sales quotations, all while ensuring a seamless experience on our portal.

The customer project manager will be our main contact at your organisation for any provisioning of products & services.


By submitting this form, the customer’s contact person acknowledges and confirms their authority to complete this form on behalf of the organization and agrees to inform Cloud One Ltd of any changes to the details provided below.

if the Authorised contact is not in the CR12 , then the Authorised Contact is duly authorised by the entity under a power of attorney to execute documents on its behalf.

The customer project manager will be our main contact at your organisation & responsible for issuing change management instructions to Cloud One
If the Project Manager & Authorised Contact is same person i.e holds both roles, then the documents must be signed by the authorised contact and a customer appointed witness to comply with Kenya's Electronic Signature Law, as governed by the Information and Communications Act.
The Customer Authorised Contact & Customer Witness MUST NOT be the same person.

For compliance with Kenya's Electronic Signature Law, as governed by the Information and Communications Act, any document requires the signatures of both the customer's authorized contact and the customer's project manager, witnessed by each other. If the same person holds both roles, then the documents must be signed by the authorised contact and a customer appointed witness.

We will set up the necessary portal contacts in our system and send the service application form to the authorized contact and project manager for their signatures. After signing the service application form, our team will create a project and share the details with the customer project manager
